TC-99m HMPAO SPECT in the assessment of cerebral complications of coronary artery bypass surgery

The importance of single photon emission computed tomography (SPECT) with 99mTc-HMPAO for the assessment of cerebral complications of aortal coronary by-passing (ACBP) done under artificial blood circulation (AC) and working heart is investigated. 45 IHD patients were involved in the study, which patients have been surgically treated for myocardial re-vascularization. AC applied surgeries were accompanied by the cerebral flow decrease in the frontal and back lobes of the right hemisphere. Patients treated with AC and Instenon and patients treated without heart stop have not demonstrated confident worsening of the brain perfusion. The cerebral perfusion changes were corresponded by the cognitive status dynamics. SPECT with 99mTc-HMPAO can be successfully applied to get quantitative evaluation of regional cerebral blood flow in ACBP patients. In such case, ACBP combined with Instenon prophylaxis or surgery at working heart prevent or significantly decrease the risk of cerebral hemodynamic disturbances and cognitive function disorders.
