Radiopharmaceuticals on the basis of fatty acids for the evaluation of myocardial metabolism

Scintigraphic studies of fatty acid metabolism in the myocardium have been reviewed. An outline has been made to characterize principal fatty acids labeled with radioiodine, which were suggested at various times for evaluation of metabolism in the heart. The most detailed consideration has been given to experimental and clinical studies of 123I-15-(p-iodophenil) pentadecanoic acid and 123I-15-(p-iodophenil)-3-R-, methylpentadecanoic acid. Reports have been evaluated on comparing the accumulation of these radiopharmaceuticals in myocardium with data from heart perfusion scintigraphy, position emission computed tomography with 11C-palmitic acid and with 18F-fluorodeoxyglucose, biopsy and contractiveness of the left ventricle. It has been shown that myocardial scintigraphy with 123I-labelled fatty acids is instrumental not only for revealing coronary deficiency with high sensitivity and specificity but also for discriminating between viable myocardial tissue and irreversibly damaged parts of the heart.
