The purpose of the study was to evaluate possible protective effect of chlorhydrate-3-ethyl-6-methyl-3-oxypiridin (emoxipine) during myocardial ischemia/reperfusion. Prior to coronary artery surgery 27 patients were randomized into 2 groups: 15 patients in 1 hour before operation received rapid intravenous infusion of emoxipine (10 ml of 3% solution) and 12 patients received infusion of placebo. Lipid peroxide concentrations; CR-MB activity and frequency of arrhythmia were evaluated. Immediately after termination of extracorporeal circulation emoxipine treated compared to placebo treated patients had lower levels of malonic dialdehyde and dienic conjugates (10,4±0,8 vs 14,9±1,5 nmol/l and 4,4±0,5 vs 5,9±0,4 nmol/l, p<0,05). Frequency of arrhythmia (25% and 47%) and defibrillation threshold (12,1±3,1 and 29,3±4,2 J, p<0,01) were also lower in emoxipine treated patients. These data indicated that emoxipine prodiced protective effect against reperfusion injury of the myocardium during open heart surgery.
